Η πρακτική της ίδιας της μεθόδου, πριν προταθεί οποιαδήποτε δράση: κάθε ριζική αιτία βαθμολογείται 1–5 για το πόσο εφικτό είναι να αντιμετωπιστεί, πόσο αντίκτυπο θα είχε η αντιμετώπισή της και πόσο πιθανό είναι να λυθεί μόνη της χωρίς παρέμβαση. Προτεραιότητα παίρνουν οι ρίζες με υψηλό αντίκτυπο, υψηλή εφικτότητα και χαμηλή πιθανότητα να διορθωθούν μόνες τους — προτεραιότητα = αντίκτυπος + εφικτότητα − πιθανότητα. Οι βαθμολογίες βγαίνουν ως μέσος όρος της ομάδας και εμφανίζονται μαζί με το πόσοι βαθμολόγησαν.

Ένα τετράγωνο που δεν κρίνει κανείς μένει ανοιχτό· δεν γίνεται ποτέ «όχι». Η κρίση κάθε ζεύγους χωριστά εμποδίζει μια δημοφιλή δράση να πάρει τα εύσημα για πράγματα που δεν αγγίζει.

Προβλέψεις για τις δράσεις: τι πρέπει να αλλάξει η καθεμία, πώς θα μετρηθεί και πότε θα ελεγχθεί. Την ημερομηνία αυτή, κάποιος που ήταν στην αίθουσα γράφει δίπλα τι συνέβη στην πραγματικότητα.

Ένας χάρτης λέει ποιες δράσεις μετακινούν ποιες αιτίες. Δεν λέει αν τις έκανε κανείς ή αν άλλαξε κάτι. Μια εγγραφή εδώ είναι πρόβλεψη με ημερομηνία και κάτι προς μέτρηση, γραμμένη αφού οριστικοποιήθηκε ο χάρτης. Όταν έρθει η ημερομηνία, γράφεται δίπλα τι συνέβη στην πραγματικότητα και η πρόβλεψη μένει όπως ήταν: μια λανθασμένη πρόβλεψη είναι η χρήσιμη. Κανείς δεν ψήφισε για τίποτα από αυτά, και δεν αλλάζει τίποτα από όσα βρήκε η αίθουσα.
